3.) This dwelling was, to the best of my/our knowledge, originally built before 1 January 2000

4.) We agree that each of you may check the accuracy and completeness of the matters in this request for assessment and we authorise any person to release to you any information about those matters for the purposes of that checking.

5.) We acknowledge that Mitre 10, PinkFit and/or EECA may contact us as part of its ENERGYWISE ™ Warmup New Zealand: Heat Smart Funding quality assurance.
